Our pricing is transparent and up front. We value your privacy so we are out of network with insurances, except straight Medicare. We work hard to keep your prices affordable and have no membership fees. Simply pay for the care you receive and nothing else. Many health insurances will reimburse you for your visit at out of network rates which generally nearly cover your costs.

Our Nurse Practitioner, Travis Prachar has been practicing in nursing since 1993. She earned her Master’s of Nursing/Family Practice in 2000 and has been practicing as a Nurse Practitioner since 2000. She has extensive experience in pediatrics, home health, cardiology, community health clinics, primary care, pulmonology, cardiothoracic surgery, and urgent care.

Starting 4/3/23, we welcomed Linda Martell who is also an autonomously licensed Nurse Practitioner. Linda has lived in the Venice community for over 2 decades and worked in nursing longer. She has worked for a local group as a primary care provider for 4 years. More recently, she has worked in a chiropractor office providing integrative care which included platelet rich plasma injection, joint injections, and medical evaluations.

To set up an appointment, please click below. Please be aware that pretreatment consent and health history forms MUST be filled out before the start of your appointment. These will be e-mailed directly to you at the moment you book the visit. Look for an e-mail from “Elation” health services, fill out all forms and submit them on-line. Please also choose to OPT IN to text messaging by texting the word “START” to 36331as this is the most efficient way to send the link for telehealth visits. In order for you to receive a copy of your visit, you will also need to sign up for our patient portal called “Passport”. This will enable easy communication between us, allow you to print any needed work/school note or release, and see the details of your care plan. You will receive a request for payment at the end of your visit.
